问题：Hello Doctor, I am 36 year old Man, I frequent massage parlours, (I know I shouldn't), but several times, over the past 6 months, I have ended up having protected vaginal sex with the CSW's. The condom always works, and I shower right after. I recently have been suffering from a sore neck . I keep checking if my lymph nodes are ok. I can't feel any unusal bumps, but my neck definitly feels unconforable. I do lift weights, maybe that is contributing to the issue. I can feel the nodes under my chin, but pretty much no where else.I don't think I have not had any other HIV symptons, such as a very sore throat,fever, rash, or thrush. Some times I feel lithargic, but I am also a diabetic, and my sugars have been climbing. Am I at high risk for HIV ? I have not checked or been tested for HIV for over a year. If the nodes which run up my neck were swollen, would they be easily felt ?? Or am I overworrying??

回复：Welcome to our Forum. there is nothing in your post to make me concerned that you are at risk for HIV. Most commercial sex workers do not have HIV, the "efficiency" of HIV transmission through UNPROTECTED vaginal sex is very low (less than one infection per every 1000 acts of intercourse), and your exposures were condom protected which make them virtually no risk, even if one of your partners did have HIV.
Bottom line, you are over worrying.You do not even need HIV testing for the exposures you describe.The problems you list are due to something other than HIV.Hope this helps.EWH

我的分析：就算有安全套保护的性行为，一般都会接触到一些阴道分泌物，或者有阴囊和阴茎根部可能有微小的伤口没注意到，所以大家比较恐，我也是恐这个。但是我认为，就算是接触到一些阴道分泌物，量也是非常小的，如果事后清洗了，也能被流动水带走大部分分泌物，从而量会更少。我觉得美国医生认为带套就是安全的，也考虑到了分泌物的问题，他们也认为伤口接触分泌物时有感染的风险的，但是他们认为量非常的少（带套不可能接触的分泌物量很大），不至于引起感染，所以在MedHelp论坛统一口径，带套是没有HIV风险的。
MEDHELP论坛对套脱落也认为不用检测，但是中国论坛什么说法都有，所以引起恐慌，因为套脱落不代表龟头还在阴道里，接触到分泌物也不多 而且时间也短 所以引不起感染 从性艾那边转过来的 不管哪里转的，帖子不错！ 就是不错不错
