Straight to your questions:-
1) no, not necessarily. The proof is in the pudding. If it was intact at the end, then all is well. The difficulty for you is that it sounds as though you don't know. It sounds though equally that she was very careful so I imagine it was. I'd be calm about that.
2) if you came into the condom then the condom was fine and you were protected. Friction results from rubbing so there would have been friction but this for you was not a risk.
3) Yours did not tear - you came into it - condom ruptures /tears are quite dramatic and you usually will feel them tear and "pop" and also see the ragged bits around the base of your penis
4) Worldwide the HIV epidemic is greatest in heterosexuals. In the industrialised world it is largest in gay and bisexual males. The follwoing play a big part in developing countries - poverty, violence, malnutrition, different sexual practices, widespread other diseases etc etc - it is complex and fills books. In most "western" countries - ie wealthy countries that picture is not replicated. The hiv populations in these countires are again confined to poor populations, gay and bisexual males, migrants etc. In teh last thirty years we have done a dreafully poor job of containing and reducing the sixe of the epidemic in wealthy countries. With respect, people like you are at actually relatively low risk.
5) yes, by a factor of between 5 to 8 times
6) well, as I say, I don't think so.
